Supported Community Living

Fast-Trans provides outreach supported community living to individuals with intellectual and developmental disabilities, brain injuries, and chronic mental illness, who are living independently in the community. Our staff work one-on-one with clients towards their daily living goals. We work with our clients to provide service while maintaining dignity and respect. It is our mission to ensure all our clients reach their goals and are active participants in the community.

Medication setup and follow through, communication with providers, and follow through on doctors recommendations.

Utilizing public transportation, community resources, understanding human rights.

Cleaning, organizing, paying bills, grocery shopping, eating, understanding nutrition, laundry.

Leisure time interests and skills, development of communication, advocacy, social interactions.

Safety

Development of safety plans, emergency preparedness, and community safety and awareness.

Our Direct Support Staff

  • Assist clients with developing coping skills, identifying patterns of behavior, and self-management skills.

  • Advance clients' decision-making, productivity, and participation in a range of activities.

  • Works collaboratively with the client to sustain their physical, mental, and emotional health.

  • Provides clients with encouragement and guidance as needed.

  • Assists clients with accessing and utilizing community resources and natural supports.

  • Builds a respectful and trusting relationship with the clients.
